We are a participating Medicare provider and will gladly submit your claims for you. We ask that you pay your 20% portion at the time the service is provided, as well as your yearly deductible if not met. We are also a participating provider of BC/BS and PPC and request that you please show your card to the receptionist. Payment for services is expected at the time they are rendered. For your convenience, we accept Mastercard and Visa as well as cash and checks. If necessary, a payment plan can be arranged with our accounting staff.
Prescription refills: When requesting a refill on your prescription, please have your medication information and the pharmacy phone number available for the office assistant. A review of your medical record by the physician will be required before refills are approved and occasionally it may be necessary for you to return to the office for a reevaluation of your condition.
