We are open to serve you, answer questions or schedule an appointment during the following hours:

Monday – Friday: 8:00 a.m. – 4:30 p.m.
*We are closed daily for lunch from 12:00 p.m. – 1:00 p.m.

Our Mission

Our mission is simple: to ensure that patients receive the treatment they need, the treatment they want and the care they deserve!


Your scheduled appointment time has been reserved specifically for you. We request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation.

Prescription Refills

When requesting a refill on your prescription, please have your medication information and the pharmacy phone number available for the office assistant. A review of your medical record by the physician will be required before refills are approved and occasionally it may be necessary for you to return to the office for a reevaluation of your condition.


Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them.

We are a participating Medicare provider and will gladly submit your claims for you. We ask that you pay your 20% portion at the time the service is provided, as well as your yearly deductible if not met. We are also a participating provider of BC/BS and request that you please show your card to the receptionist. Payment for services is expected at the time they are rendered. Read more.
