Before you arrive:

Your first visit to a dermatologist is important, and it will be much easier for both you and our office if you are well-prepared. Make a concrete list of issues or concerns that you want addressed. Also take some time to make a detailed history of your existing skin conditions:

 

  • When did you first notice the condition?

  • Has it gotten larger or more prominent since?

  • Have you noticed any changes in color, size, shape, frequency, etc. of the condition?

You should also bring along a list of any medications and skincare products that you are currently using. Please arrive at least 15 minutes early for your first visit to give yourself time to fill out any needed paperwork. For your convenience, you can download, print and fill out your needed patient forms prior to your arrival.

Depending on your skin conditions and concerns, your first visit could last anywhere from 15 minutes to an hour. This appointment is a great opportunity to ask questions, learn about possible treatment options and get reasonable expectations for how much your condition can be treated – you may be surprised at the amount of skin conditions you don't have to live with, thanks to the advances in dermatology! For your convenience, you may download the below Patient Forms and email them to the Lecanto or Ocala office. To do so, first you must download the forms, enter your information, save to your computer, and then email to the respective office below.

We are open to serve you, answer questions or schedule an appointment during the following hours:

Monday – Friday: 8:00 a.m. – 4:30 p.m.

Appointments

Your scheduled appointment time has been reserved specifically for you. We request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation.

Prescription Refills

When requesting a refill on your prescription, please have your medication information and the pharmacy phone number available for the office assistant. A review of your medical record by the physician will be required before refills are approved and occasionally it may be necessary for you to return to the office for a reevaluation of your condition.

Insurance

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them.

We are a participating Medicare provider and will gladly submit your claims for you. We ask that you pay your 20% portion at the time the service is provided, as well as your yearly deductible if not met. We are also a participating provider of BC/BS and request that you please show your card to the receptionist. Payment for services is expected at the time they are rendered. Read more.
